Helping Ukrainian families and their hosts access NHS services.

 

With many households in Worcestershire supporting displaced families as part of the Homes for Ukraine programme, we’ve created resources to help both new arrivals and the families hosting them to make the most of the NHS services available in our local area.

First of all, we’ve developed a new How to Access the NHS in Worcestershire guide in both English and Ukrainian.

We recommend host families read through the English version so they’re up-to-date with the way NHS services are being delivered locally - as a combination of both new services in our area, plus changes due to Covid-19, mean there may be new services you and those staying with you are able to use.

We’ve also created translated versions of the GP registration forms, which all new arrivals will need to complete in order to register with a GP.

These include the GMS1 form, required to register at any GP practice in England, and a generic New Patient Questionnaire, which should answer most of the questions your practice will need to ask as part of their initial patient screening when you join.

Your local Primary Care Network or council may recommend Ukrainian families register at a particular practice in your area. If this is the case, your contacts at your local council will let you know.

You can download the relevant forms from this page and take them with you to your GP practice when registering, if they’re not already provided in your local council’s welcome pack.

    Each GP practice has its own New Patient Questionnaire - so your surgery may ask additional questions beyond those covered here.

    However, this generic form is designed to cover most of the questions your practice will ask as part of your initial registration in both English and Ukrainian.
