This publication was updated on 25 February 2020. The p-values presented for ethnicity in Table D2 and the labelling of Table A1 have been corrected. Previously we reported in Table A1 that the unexplained (pp) was calculated using the ‘full’ mixed-effect logistic regression model when in fact the ‘simplified’ model was used.
