We know that even though many end of life patients have conditions that will not be helped by going to an acute hospital, many of them will end up being taken there when they become less well - this puts increasing pressure on NHS resources and is also often not the preferred place of care for people who are at the end of their life. The PEACE document (Proactive Elderly Advance Care Plan) has helped increase the number of people who spend their last days in the familiarity of their own home, or nursing home.
In this talk Dr Jonathan Riordan will take you through the principles of the PEACE document and how to use it to deliver the best care to people with a life limiting illness. This talk is aimed at professionals working in primary care settings and care homes as well as GPs and nurses working in the community.
Dr Jonathan Riordan, Consultant in Palliative Medicine, will be delivering the masterclass. Dr Riordan completed medical training in the West Midlands, London and Queensland Australia before completing 5 years of specialist palliative care training in a number of hospitals and hospices around Essex. He joined St Clare Hospice in November 2019.
