About the Home First, Act Now programme
• Aims and ambitions:
– The aim of this elearning programme is to increase awareness around Home First Principles in the Discharge Policy. The programme supports health and care professionals involved in the discharge process, to act in a way that values patient time and helps facilitate safe and timely discharge.
• How learners will benefit from doing this course
– By completing the sessions, learners will understand the principles of Home First, see first hand case study examples and share best practice.
• Who it’s aimed at:
– Home First, Act Now is suitable for a range of health and care professionals including nurses, AHPs, care staff and students across NHS providers, commissioners and social care.
• Completing the programme:
-Accessibility to the programme is very easy – you need to have an eLearning for Health (elfh) account and registration for this can be from any email address (including personal emails)
• Course content
– Section 1: The principles of Home First
– Section 2: Working together
– Section 3: Valuing people’s time
– Duration: 62 minutes plus 62 minutes of optional content
– Assessment (if relevant): Self assessment questionnaire

How to access
In order to access the Home First, Act Now programme, you will need an elfh account. If you do not have one, then you can register by selecting the Register button below.
To view the Home First, Act Now programme, select the View button below. If you already have an account with elfh, you will also be able to login and enrol on the programme from the View button.
Not an NHS organisation?
If you are not an NHS health or care organisation and therefore do not qualify for free access elfh Hub, you may be able to access the service by creating an OpenAthens account.
To check whether or not you qualify for free access via OpenAthens, you can view the eligibility criteria and register on the ‘OpenAthens’ portal.
Registering large numbers of users
If you are a HR, IT or Practice Manager and would like to register and enrol large numbers of staff within your organisation for access onto the Home First, Act Now programme, please contact elfh directly.
Organisations wishing to use their own LMS
