To access Hamad Medical Corporation and Primary Health Care Corporation hospitals and centers it is
recommended that you apply for a health card. The health card entitles the holder to subsidized consultations
and prescriptions. People with private medical insurance are also encouraged to apply for a health card in
case they need to visit one of the government facilities.
To get a health card, residents must visit the Primary Health Center nearest to where you live – call 107 if you
are unsure where your local center is – and bring with you the below documents according to your needs:
• Everyone: A copy of your utility
bill (phone, water/electricity) – this
proves where you live.
• Qatari Adults: Copies of your
passport or ID, a passport-size
photograph and a fee of QR50 (to
be paid with a credit card).
• Qatari Children: Birth certificate
with Qatar personal identity
number or passport, passport-size
photograph, vaccination card (for
first time issue) and a fee of QR50
(to be paid with a credit card).
• GCC National Adult Residents:
Copy of passport or ID, a
passport-size photograph and a fee
of QR50 (to be paid with a credit
card).
• GCC National Child Residents:
Birth certificate or passport, one
passport-size photograph,
vaccination card (for first time issue
for children born in Qatar), and fees
of QR50 (to be paid with a credit
card).
• Adult Residents: Valid QID or
passport with valid residence
permit, a passport-size
photographs and a fee of QR100
(to be paid with a credit card).
• Children: Passport with valid
residence permit, a passport-size
photograph, vaccination card (for
first time issue and for those born in
Qatar or who had their vaccinations
in Qatar) and a fee of QR100 (to be
paid with a credit card).
• Domestic Staff: Valid passport
with a valid residence permit, a
passport-size photograph, original
QID of sponsor and a fee of QR50
(to be paid with a credit card), as for
professions approved by MOI for
personal sponsorship.
At the health center you need to fill out the appropriate forms. You will be issued a PHCC health file number.
The health center will either issue you with your card immediately or ask you to return to collect it.
