This survey is voluntary, and you do not need to take part. By completing this survey, you are giving us permission to use this information to inform our plans. Your individual services will not be affected by taking part or choosing not to.
The purpose of this questionnaire is to gain the views from Essex Residents aged 65+ who have used day centres.
We would like you to be able to contribute your ideas to help shape our future day opportunities provision.
